Summary

Gastric surgery, specifically Pólya partial gastrectomy, does not increase colorectal cancer risk in rats. This procedure may even offer protection by reducing colonic cell proliferation and tumor development.

Background:

  • Peptic ulcer surgery is suspected to increase colorectal cancer risk.
  • The mechanisms linking gastric operations to colonic carcinogenesis are not fully understood.

Purpose of the Study:

  • To investigate the effects of different gastric operations on colonic cell proliferation, bile acid excretion, and colorectal carcinogenesis in a rat model.
  • To evaluate whether Pólya partial gastrectomy or vagotomy/pyloroplasty influences colorectal tumor development.

Main Methods:

  • 105 male Sprague-Dawley rats underwent sham operation, Pólya partial gastrectomy, or vagotomy and pyloroplasty.
  • Azoxymethane was administered to induce colorectal cancer.
  • Colonic tissues were analyzed for mucosal mass and crypt cell production rate (CCPR).

  • Fecal samples were assayed for neutral steroids and bile acids.
  • Main Results:

    • Pólya gastrectomy significantly reduced CCPR (42-65%) and the incidence of colorectal tumors (26% vs. 63%).
    • Gastric surgery decreased fecal levels of neutral steroids and bile acids, including hyodeoxycholic acid.
    • Vagotomy and pyloroplasty increased caecal CCPR but did not alter tumor yield or significantly affect fecal steroid levels.

    Conclusions:

    • The study results do not support the hypothesis that gastric surgery predisposes to colonic carcinogenesis.
    • Pólya partial gastrectomy appears to exert a protective effect against colorectal cancer development in rats, likely by inhibiting colonic cell proliferation.
