    Summary

    Incidental gallbladder carcinoma, diagnosed in 0.5% of patients undergoing cholecystectomy for gallstones, is often detected early. Early detection through cholecystectomy for gallstones improves prognosis.

    Background:

    • Gallbladder carcinoma is rare with a poor prognosis.
    • Laparoscopic cholecystectomy for benign conditions increases incidental gallbladder cancer detection.
    • Early detection improves patient outcomes.

    Purpose of the Study:

    • To report the incidence and management of incidentally diagnosed gallbladder carcinoma.
    • To analyze outcomes of gallbladder cancer detected during or after laparoscopic cholecystectomy.
    • To evaluate the role of cholecystectomy for gallstones in detecting early-stage gallbladder cancer.

    Main Methods:

    • Retrospective analysis of 1193 patients undergoing cholecystectomy (Jan 2003-Dec 2011).
    • Pathological examination of surgical specimens for adenocarcinoma.
    • Correlation of incidental findings with intraoperative observations and frozen section analysis.

    Main Results:

    • Gallbladder adenocarcinoma was found in 6 of 1188 patients (0.5%).
    • No pre-operative suspicion of malignancy in these cases.
    • Tumor stages ranged from T1 to T3; only T1 underwent simple cholecystectomy.

    Conclusions:

    • The incidence of incidental gallbladder carcinoma is 0.5%, consistent with literature.
    • Gallstones and advanced age are risk factors for gallbladder cancer.
    • Cholecystectomy for gallstones is indicated, potentially leading to early cancer detection.
