Psychosis: Goals of Pharmacotherapy

Antipsychotic drugs are a crucial treatment method for acute and chronic psychoses, bipolar illness, and behavioral disorders. The selection of these drugs depends on several factors, including the state of the disease, clinical judgment, possible drug interactions, and the patient's sensitivity to adverse effects. In immediate scenarios, such as delirium and dementia, short-term treatment with low doses of high-potency typical or atypical agents can effectively manage symptom exacerbation. For...

Psychosis and Antipsychotic Drugs: Overview

The term "psychosis" refers to a spectrum of mental disorders characterized by abnormal thoughts, perceptions, and behaviors. It can manifest as mood disorders, dementia, delirium with psychotic features, substance-induced psychosis with psychotic features, brief psychotic disorder, delusional disorder, schizoaffective disorder, and schizophrenia. Among all these disorders, schizophrenia is the most common psychotic disorder, affecting 1% of the worldwide population. Psychotic symptoms in all...

Summary

Experience sampling methodology (ESM) is a feasible and valid tool for tracking fluctuating psychotic symptoms in patients starting antipsychotic treatment. This method captures detailed changes often missed by standard clinical interviews.

Area of Science:

  • Psychiatry
  • Clinical Psychology
  • Neuroscience

Background:

  • Acute psychosis presents challenges in symptom monitoring.
  • Traditional symptom assessment may not capture dynamic symptom fluctuations.
  • Experience sampling methodology (ESM) offers a potential solution for real-time symptom tracking.

Purpose of the Study:

  • To assess the feasibility and validity of using ESM for measuring temporal changes in psychotic symptoms.
  • To evaluate ESM's utility in patients with acute psychosis initiating antipsychotic treatment.

Main Methods:

  • Twenty-six inpatients with delusions were monitored using ESM on a personal digital assistant (PDA) seven times daily for 14 days.
  • Standardized symptom interviews were conducted at baseline, 1 week, and 2 weeks.
  • Feasibility assessed by completion rates and compliance; validity by comparing ESM data with clinical ratings.

Main Results:

  • A 61.5% completion rate and 70.7% compliance were observed.
  • ESM showed internal consistency and feasibility in acute psychosis.
  • ESM and clinical ratings converged on suspiciousness and images but diverged on auditory hallucinations and most delusion dimensions.

Conclusions:

  • ESM is a feasible and internally valid method for monitoring psychotic symptoms during acute episodes.
  • ESM provides rich, dynamic symptom change data potentially missed by observer ratings.
  • Further research can explore ESM's role in treatment response assessment.
