Background:

Inferior alveolar nerve (IAN) damage is a serious complication in implantology.
The mandibular incisive nerve, a terminal branch of the IAN, innervates anterior teeth.
Damage to the incisive nerve and canal, located in the interforaminal area, is underreported.

Purpose of the Study:

To evaluate the risk of neuropathic pain from dental implants placed in the mandibular interforaminal region.
To investigate the relationship between implant position and incisive nerve damage.

Main Methods:

Retrospective analysis of panoramic radiographs from 55 patients with suspected incisive canal-implant proximity.
3D analysis of CT scans from 10 patients with postoperative neuropathic pain.

Evaluation of implant-nerve relationship using specialized software.

Main Results:

Mandibular incisive nerve perforation by at least one implant was confirmed in all 10 patients with neuropathic pain.
Implant placement in the interforaminal region poses a risk of incisive canal perforation.

Conclusions:

Implant placement in the mandibular anterior region can lead to incisive canal and nerve perforation.
This perforation is a potential cause of postoperative neuropathic pain.
Incisive canal and nerve perforation should be recognized as a complication of mandibular anterior implant surgery.
