Summary

Venous sclerosis (phlebosclerosis) is underreported. In a patient needing bypass surgery, veins from an atrophic leg were unexpectedly healthy and suitable for grafting, unlike those from a healthy limb.

Observation:

  • A 72-year-old male patient with a paralyzed, atrophic right leg (due to polio) required coronary artery bypass surgery.
  • The great saphenous vein from his left (healthy) leg was found to be a hardened, obliterated cord.
  • Unexpectedly, veins harvested from the patient's atrophic right leg were normal and suitable for surgical grafting.

Findings:

  • The study highlights a paradoxical finding: veins from an atrophic limb were healthy, while veins from a seemingly normal limb showed signs of sclerosis.
  • This case challenges assumptions about venous health in limbs affected by disuse atrophy.
  • The harvested veins from the atrophic limb were successfully utilized for coronary artery bypass grafting.

Implications:

  • This case suggests that disuse atrophy might preserve venous health, contrary to expectations.
