Background:

  • The calcifying odontogenic cyst (COC) is a rare odontogenic tumor.
  • A significant association exists between COC and odontoma (24% of cases).
  • Understanding calcifying odontogenic cyst associated with odontoma (COCaO) may clarify its pathogenesis.

Observation:

  • A review identified 52 cases of COCaO.
  • The mean age was 16 years, with a 1:1.9 male to female ratio.
  • The maxilla was the most common site (61.5%), with lesions typically appearing as well-defined, mixed radiolucent-radiopaque masses.

Findings:

  • Histologically, COCaO presents as a single large cyst with integral tooth-like structures.
  • Distinct differences in gender distribution and location were noted compared to simple COC.
  • COCaO exhibits unique histologic features differentiating it from simple COC.

Implications:

  • COCaO may represent a distinct entity within odontogenic lesions.
  • Classification as a benign, mixed odontogenic tumor is proposed for COCaO.
  • The term 'odontocalcifying odontogenic cyst' is suggested for this specific entity.
