Two patients with amyloid goitre, a rare thyroid enlargement, were studied. Despite appearing euthyroid, their thyroid function tests indicated thyrotoxicosis due to primary systemic amyloidosis.

Area of Science:

  • Endocrinology
  • Pathology

Background:

  • Primary systemic amyloidosis is a rare condition characterized by amyloid protein deposition in various organs.
  • Thyroid involvement can occur but is infrequently reported, particularly presenting as amyloid goitre.

Observation:

  • Two cases of amyloid goitre secondary to primary systemic amyloidosis are presented.
  • Both patients were clinically euthyroid, showing no overt signs of hyperthyroidism.

Findings:

  • Post-mortem confirmation of primary systemic amyloidosis was established in both patients.
  • Thyroid function tests revealed hyperthyroxinaemia and other abnormalities suggestive of thyrotoxicosis, despite the euthyroid clinical state.

Implications:

  • This presentation highlights a rare manifestation of systemic amyloidosis affecting the thyroid gland.

Related Experiment Videos

  • The discrepancy between clinical euthyroidism and biochemical thyrotoxicosis in amyloid goitre warrants further investigation and awareness among clinicians.
