Background:

Epidermoid cysts (atheromas) typically arise from pilosebaceous units.
Cysts in hairless areas like the palm are uncommon.
Previous theories linked their development to trauma or HPV-60 infection.

Purpose of the Study:

To report a rare case of a large epidermoid cyst in the palm.
To explore potential causative mechanisms in hairless regions.

Main Methods:

A 93-year-old male presented with a large palm mass.
Diagnosis was confirmed via incisional biopsy.
Surgical debridement and resection were performed after rupture and infection.

Main Results:

Pathological diagnosis confirmed an epidermoid cyst.
The patient experienced a favorable postoperative recovery.
This represents the largest reported epidermoid cyst in the palm.

Conclusions:

Epidermoid cyst development in hairless areas is rare and poorly understood.
While trauma and HPV-60 are known factors, chronic loading may also contribute.
This case highlights alternative etiologies for epidermoid cysts in unusual locations.
