Multiple Sclerosis l: Introduction

Multiple sclerosis is a chronic autoimmune disease of the central nervous system (CNS) that affects the brain, spinal cord, and optic nerves. It is an inflammatory demyelinating disorder and a leading cause of neurological disability in young adults.EpidemiologyMS commonly begins between 20 and 40 years of age and is twice as common in women. Its exact cause remains unclear, but genetic susceptibility contributes, with higher risk in first-degree relatives and identical twins. A greater...

Summary
This summary is machine-generated.

Tumefactive multiple sclerosis (MS) is rare in Taiwan, often affecting women initially. Despite severe initial lesions, these patients show a better prognosis than expected, with less optic/spinal involvement.

Area of Science:

  • Neurology
  • Neuroimmunology
  • Demyelinating Diseases

Background:

  • Multiple sclerosis (MS) prevalence and characteristics vary geographically, being less common in Asia.
  • Tumefactive demyelinating lesions are an exceptionally rare presentation of MS.

Purpose of the Study:

  • To review cases of tumefactive MS in Taiwan.
  • To compare these cases with international studies on tumefactive demyelinating lesions and local typical MS patients.

Main Methods:

  • Retrospective review of 190 MS patients from 1985-2010.
  • Inclusion criteria: MS diagnosis (McDonald/Poser criteria) and brain lesions >2cm with potential mass effect.

Main Results:

  • 12 patients (6.3%) had tumefactive MS; 91.7% were female, presenting lesions at first attack.
  • Relapsing-remitting course with a second attack within 2 years.
  • Favorable prognosis (>2 years follow-up) despite higher initial disability; absence of optic/spinal involvement and specific antibodies.

Conclusions:

  • Tumefactive MS is uncommon in Taiwan.
  • Initial tumefactive demyelinating lesions, though severe, have a relatively favorable long-term prognosis.
