Background:

Frotteurism, a paraphilic disorder, involves recurrent intense sexual arousal from touching or rubbing against a non-consenting person.
While common and often overlooked, public frottage carries legal consequences.
Manifestations of frotteurism alongside psychiatric illness or developmental abnormalities warrant careful consideration of legal and clinical implications.

Observation:

A 12-year-old boy presented with unrestrained, impulsive frotteuristic behavior towards peers and his younger brother.
He experienced distress and guilt due to his actions but lacked self-control.
This behavior was linked to an underlying concealed psychiatric condition, including depression and obsessive-compulsive disorder.

Findings:

The patient exhibited hypersexual behavior, including masturbation with frotteuristic elements, leading to the development of depression.
This compulsive frotteurism coupled with severe depression is a rare clinical presentation.
Treatment with Sertraline, psychiatric counseling, and rehabilitation resulted in complete symptom remission within six months without relapse.

Implications:

This case underscores the importance of recognizing and managing hypersexual behavior in adolescents, particularly when comorbid with psychiatric conditions.
It highlights the diagnostic challenges and therapeutic approaches for frotteurism in the context of neurodevelopmental and psychiatric disorders.
Understanding the interplay between paraphilias, psychiatric illness, and legal liability is crucial for comprehensive patient care and forensic evaluations.
