Summary

The Increasing Stability Through Evaluation and Practice (InSTEP) program effectively reduced falls in older adults. The medium-intensity InSTEP model showed significant improvements in self-reported falls and overall fall risk reduction.

Area of Science:

  • Gerontology
  • Public Health
  • Rehabilitation Science

Background:

  • Falls are a significant threat to the health and independence of community-dwelling older adults.
  • Existing fall prevention programs often lack comprehensive, adaptable models.
  • The Fall Prevention Center of Excellence sought to develop and evaluate innovative fall prevention strategies.

Purpose of the Study:

  • To design and assess the effectiveness of three progressive-intensity fall prevention program models, known as Increasing Stability Through Evaluation and Practice (InSTEP).
  • To reduce fall risk factors and actual fall rates in community-dwelling older adults.
  • To compare the efficacy of different intensity levels within the InSTEP program.

Main Methods:

  • A within-subjects design with 200 participants was employed, measuring outcomes at baseline, postintervention, and at 3 and 9 months.
  • The InSTEP program models integrated physical activity, medical risk management, and home safety components over 12 weeks.
  • Fall rates and risk factors were assessed using performance tests, self-reported function, and fall diaries.

Main Results:

  • Participants demonstrated an overall reduction in falls and improved self-perceived gait and balance.
  • Significant improvements in dynamic gait function were observed across participants.
  • The medium-intensity InSTEP model significantly reduced self-reported falls (p = .003) compared to other models.

Conclusions:

  • The InSTEP program is a feasible and effective model for reducing fall risk in older adults living in the community.
  • The medium-intensity InSTEP program shows particular promise for reducing self-reported falls.
  • Comprehensive, multi-component interventions are crucial for addressing fall prevention in aging populations.
