Professionalism is a core competency in medical education.
Understanding resident perspectives on professionalism development is crucial for effective training.

Purpose of the Study:

To characterize and understand pediatric emergency medicine residents' perspectives on professionalism development.
To explore how experiences in pediatric emergency medicine shape resident understanding of professionalism.

Main Methods:

Qualitative study using freelisting and semistructured interviews with senior residents.
Analysis of word saliency and thematic consensus from interview transcripts.

Main Results:

Residents associated professionalism with "respect," "compassion," "empathy," and "integrity," but lacked consensus.
Professionalism development was described through observation, interactions, and environment, with role modeling being most influential.
Residents reported limited direct feedback and observation from attending physicians regarding professionalism.

Conclusions:

Significant variability exists in residents' understanding of professionalism, often differing from ACGME competency definitions.
Resident perspectives on professionalism can inform refinements to ACGME milestones and entrustable professional activities.
