Exercise stress testing, commonly known as a treadmill test, is a noninvasive procedure used to evaluate cardiovascular function and diagnose heart conditions.
Definition
An exercise stress test measures the heart's response to exertion using a treadmill or stationary bicycle. Chest electrodes record the heart's electrical activity through an ECG, and blood pressure is monitored regularly.

Background:

  • Postconcussion syndrome (PCS) is poorly understood regarding exercise intolerance.
  • The effectiveness of exercise evaluations in PCS management is unclear.

Purpose of the Study:

  • To evaluate exercise intolerance in male and female patients diagnosed with PCS.
  • To assess the utility of exercise testing as a monitoring tool for PCS recovery.

Main Methods:

  • A cross-sectional laboratory study compared 34 PCS patients with 22 healthy controls.
  • Participants underwent a graded treadmill test (Balke protocol) measuring symptoms, heart rate, blood pressure, and perceived exertion.
  • Exercise cessation occurred due to symptom exacerbation or inability to continue.

Main Results:

  • PCS patients exhibited significantly shorter exercise duration (8.5 vs. 17.9 min) and lower peak heart rate and systolic blood pressure compared to controls.
  • The odds of a shorter exercise duration were nearly 8 times greater in the PCS group.
  • Perceived exertion was lower in PCS patients, while systolic blood pressure showed a sex difference within the PCS group.

Conclusions:

  • Individuals with PCS demonstrate a symptom-limited response to exercise.
  • Graded treadmill testing appears to be a valuable tool for monitoring recovery progress in patients with PCS.
