Background:

  • Retrobulbar hemorrhage is a serious, sight-threatening complication following orbital decompression surgery.
  • Orbital compartment syndrome can lead to rapid vision loss if not treated promptly.

Observation:

  • A patient developed sudden retrobulbar hemorrhage 36 hours post-surgery while forcefully biting.
  • The hemorrhage originated from the temporalis muscle, causing orbital compartment syndrome.

Findings:

  • Rapid surgical intervention, including canthotomy and cantholysis, was performed.
  • Vision recovered from absent light perception to 20/20 following the intervention.

Implications:

  • This case describes a novel cause of retrobulbar hemorrhage induced by mastication.
  • Early recognition and intervention are crucial for preserving vision in such cases.
