Background:

Accurate prediction of post-operative refractive error is a significant challenge in cataract surgery.
Intraocular lens (IOL) power calculation errors can lead to suboptimal visual outcomes.

Purpose of the Study:

To evaluate the utility of intraoperative autorefractometry (IOAR) in preventing post-operative refractive errors.
To demonstrate the successful application of IOAR in a clinical setting for immediate IOL power adjustment.

Main Methods:

A case study involving an 83-year-old female patient undergoing phacoemulsification with IOL implantation.
IOAR was utilized after initial IOL insertion to assess refractive error.
An IOL exchange was performed based on IOAR findings before completing the surgery.

Main Results:

The initial IOAR measurement indicated a significant hyperopic shift (+1.1D) from the targeted refraction.
An IOL exchange to a different power was performed.
The second IOAR measurement confirmed a refractive outcome close to the target (-0.13D).
The patient achieved a final refraction of -0.25D three years post-operatively.

Conclusions:

Intraoperative autorefractometry is a valuable tool for real-time refractive error detection during cataract surgery.
IOAR enables immediate correction of IOL power, thereby avoiding post-operative refractive surprises and improving patient satisfaction.
