Recurrent aorto-duodenal fistula, a rare complication after aortic graft surgery, was successfully treated by repositioning the duodenum. This ante-colic approach offers a novel solution for managing this challenging condition.

Background:

  • Secondary aorto-duodenal fistula is a rare but serious complication (<1%) following prosthetic abdominal aortic graft placement.
  • Recurrence rates after initial fistula repair can be as high as 47%, posing significant management challenges.

  • A case of recurrent aorto-duodenal fistula is presented.
  • The patient had previously undergone treatment for this condition.

Findings:

  • Successful treatment was achieved by re-sitting the duodenum in an ante-colic position.
  • This surgical repositioning represents an alternative management strategy.

Implications:

  • This ante-colic repositioning technique may offer a viable solution for recurrent aorto-duodenal fistulas.
  • Further studies are warranted to evaluate the long-term efficacy and safety of this approach.
