  • Root canals were instrumented using Mtwo, Twisted File, or WaveOne systems.
  • Microbiological samples were collected pre- and post-instrumentation, with irrigation using distilled water or 5.25% NaOCl.
  • Bacterial regrowth was monitored for 60 days using Kaplan-Meier survival analysis.

    • All systems achieved high initial percentage reduction (95.9%–100%) of E. faecalis, with no significant differences between rotary and reciprocating systems when using either irrigant.
    • In the sodium hypochlorite groups, the Mtwo system showed significantly less bacterial regrowth at 60 days compared to Twisted File (P=0.029) and WaveOne (P=0.005).

    Conclusions:

    • Both rotary and reciprocating systems effectively reduce E. faecalis in root canals.
    • The Mtwo system exhibits superior long-term disinfection efficacy, indicated by reduced bacterial regrowth over 60 days.
    • Sodium hypochlorite irrigation enhances disinfection, but system choice influences long-term outcomes.
