This study found that while duodenal small bowel carcinoid (SBC) showed better survival initially, the primary tumor site was not a significant long-term predictor. Other factors like age and tumor stage are more crucial for predicting survival in SBC patients.

Area of Science:

  • Oncology
  • Gastroenterology
  • Epidemiology

Background:

  • Small bowel carcinoid (SBC) is a rare neuroendocrine tumor.
  • The prognostic significance of the primary tumor site in SBC remains debated.

Purpose of the Study:

  • To investigate the prognostic significance of the primary site of disease for small bowel carcinoid (SBC).
  • To analyze survival data using a large, population-based cohort.

Main Methods:

  • Utilized the Surveillance, Epidemiology and End Results (SEER) database (1988-2009).
  • Analyzed overall survival (OS) and disease-specific survival (DSS) using Kaplan-Meier and Cox regression models.
  • Adjusted for factors including age, tumor stage, and lymph node status.

  • Duodenal SBC primaries showed initially better 10-year OS and DSS compared to jejunal and ileal primaries.
  • However, multivariate analysis revealed primary site location was NOT a significant predictor of survival.
  • Age, number of primaries, lymph node examination, T-stage, and M-stage were independent predictors of survival.

Conclusions:

  • The primary site of small bowel carcinoid (SBC) alone is not a significant predictor of long-term survival.
  • Clinical factors such as age and tumor stage are more critical for prognostic assessment in SBC.
