Transition Zone01:28

Transition Zone

The transition zone in concrete is a critical area where aggregate meets cement paste, marked by a distinct porosity and weakness compared to the surrounding material. The adhesion around the aggregates is primarily due to Van Der Waals forces. The voids within this zone influence its robustness; initially, it is less durable than the surrounding bulk mortar due to larger voids. Initially, when concrete is compacted, a higher water-cement ratio near the aggregates leads to the formation of...
Requirements for Human Life01:26

Requirements for Human Life

The Earth and its atmosphere have provided humans with air, water, and food, but these are not the only requirements for survival. Humans also require a specific range of temperature and pressure that the Earth and its atmosphere provides.
Oxygen
Atmospheric air is only about 20 percent oxygen, but that oxygen is a key component of the chemical reactions that keep the body alive, including the reactions that produce ATP. Brain cells are susceptible to a lack of oxygen because they require a...
Limits at Infinity01:24

Limits at Infinity

The function that decreases as the input becomes very large provides a clear example of how mathematical functions can behave at extreme values. When the input increases continuously, the output becomes smaller and smaller, getting closer to a particular fixed value. Although the output never actually reaches this value, it moves nearer to it without limit. This behavior is a fundamental concept in understanding how functions behave as the input grows indefinitely. The graphical representation...
Hybrid Zones02:29

Hybrid Zones

Hybrid zones are narrow regions where two closely related species interact, mate, and produce hybrids. Relative to either parent species, hybrids may possess distinct phenotypic or genetic differences that impact their survival and reproductive success. The genetic variances introduced by hybridization influence species diversity and speciation processes within the hybrid zone.
Zones of Protection01:16

Zones of Protection

In power systems, the entire setup is divided into protective zones to isolate faults and protect the rest of the network. These zones include generators, transformers, buses, transmission lines, distribution lines, and motors. Each zone can be visualized as a separate room in a house, with each room protected by its own circuit breaker.
Protective zones are defined by closed dashed lines, containing one or more components. A key characteristic of these zones is the strategic placement of...
Altered States of Awareness01:06

Altered States of Awareness

Altered states of consciousness represent significant deviations from one's normal mental state. These deviations can range from subtle changes in awareness to profound transformations in perception, thought processes, and sensory experiences. Altered states of consciousness can be triggered by various factors, including drug use, meditation, hypnosis, illness, or even intense fatigue.
The ingestion of substances like stimulants or hallucinogens leads to chemical alterations in the brain that...

Summary

The ACOSOG Z0011 trial found that some breast cancer patients with limited axillary disease did not benefit from axillary lymph node dissection (ALND) when receiving radiation and systemic therapy. Continued long-term follow-up is crucial for this cohort.

Area of Science:

  • Oncology
  • Surgical Oncology
  • Radiotherapy

Background:

  • The ACOSOG Z0011 (Z11) trial investigated the role of axillary lymph node dissection (ALND) in breast cancer management.
  • Results indicated a subset of patients with limited axillary disease may not benefit from ALND when treated with whole breast radiotherapy and systemic therapy.

Purpose of the Study:

  • To emphasize the importance of continued long-term follow-up for patients meeting Z11 criteria who did not undergo ALND.
  • To advocate for prospective data collection on this cohort to monitor for late survival differences.
  • To encourage future research to be inclusive of patient groups excluded from the original Z11 trial.

Main Methods:

  • Analysis of outcomes from the ACOSOG Z0011 trial cohort.
  • Recommendation for prospective data recording on patients managed without ALND.
Keywords:

  • Call for inclusive future clinical trials in breast cancer axillary staging.
  • Main Results:

    • Short-term follow-up suggests no benefit from ALND in specific breast cancer patient groups under multimodal therapy.
    • The need for long-term surveillance of patients managed conservatively regarding axillary staging.

    Conclusions:

    • Patients fitting Z11 criteria with limited axillary disease may not require ALND if receiving whole breast radiotherapy and systemic therapy.
    • Long-term outcomes and prospective data collection are essential for this patient group.
    • Future studies should broaden inclusion criteria beyond Z11 to enhance generalizability.
