Summary

Deep brain stimulation (DBS) can cause rare cysts at electrode tips, even without infection. Removing the affected DBS lead resolves these cysts and associated neurological deficits.

Observation:

  • Two patients developed neurological deficits months after DBS surgery.
  • Imaging revealed cyst formation at the tip of the DBS electrode in both cases.
  • Cysts were unilateral and not associated with infection.

Findings:

  • Ventral intermediate nucleus lead for essential tremor and subthalamic nucleus leads for Parkinson's disease were implicated.
  • Removal of the affected DBS lead led to rapid symptom regression.
