Background:

  • Donor lymphocyte infusion (DLI) is a standard treatment for leukemia relapse after stem cell transplantation.
  • However, DLI efficacy is limited in advanced relapse, with a high risk of severe graft-versus-host disease (GvHD).

Purpose of the Study:

  • To develop and evaluate a novel risk-adapted DLI strategy for improving outcomes in patients with mixed chimerism post-stem cell transplantation.
  • To minimize leukemia relapse and GvHD while maximizing overall survival.

Main Methods:

  • A risk-adapted DLI strategy was implemented, involving pre-emptive DLI at mixed chimerism, dose adjustments based on donor type, real-time chimerism monitoring, and DLI cessation upon achieving full chimerism.
  • The study included 38 patients with mixed chimerism who received 178 DLIs.

Main Results:

  • Eighty-six-point-eight percent (33/38) of patients achieved increased donor chimerism, with 78.9% (30/38) reaching 100% donor chimerism.
  • Patients with fully corrected chimerism showed a significantly lower cumulative incidence of relapse (P=0.00004) and higher overall survival (P=0.0003) compared to those with mixed chimerism.
  • Only 13.2% of patients developed acute grade III-IV GvHD, with no associated mortality.

  • The risk-adapted DLI strategy effectively converts mixed chimerism to full donor chimerism.
  • This approach significantly reduces the risk of leukemia relapse, GvHD, and mortality in patients undergoing stem cell transplantation.
  • The strategy offers a safer and more effective method for managing mixed chimerism post-transplant.
