Summary

Military reconstructive surgeons faced complex extremity injuries from recent wars. Multiple free flaps for limb salvage had higher complication rates but did not increase flap or limb failure in carefully selected patients.

Background:

  • The Iraq and Afghanistan Wars led to numerous challenging extremity injuries.
  • Military reconstructive surgeons encountered cases requiring multiple tissue transfers for limb salvage.
  • This study examines microsurgical techniques for war-injured patients.

Observation:

  • A review of 74 free flap limb salvage cases from 2003-2012 was conducted.
  • Four patients required simultaneous free tissue transfers for separate upper and lower extremity injuries.
  • Complication rates were compared between single and multiple free-flap procedures.

Findings:

  • Multiple free flaps (two per patient) had a 63% complication rate versus 26% for single flaps (p=0.046).
  • Despite higher complications, multiple free flaps did not increase flap or limb salvage failure rates (p=0.892 and 0.626).
