Background:

  • Hyper IgE syndrome (HIES), also known as Job's syndrome, is a rare primary immunodeficiency.
  • Characterized by elevated serum IgE, eczema, and recurrent infections of the skin and lungs.

Observation:

  • A 16-year-old boy presented with a year-long history of skin lesions on his knees and elbows, initially diagnosed as psoriasis.
  • He had a history of recurrent infections, including otitis media, pneumonia, diarrhea, and skin infections.
  • Laboratory tests revealed elevated total IgE levels with normal immunoglobulin levels.

Findings:

  • Histological examination of skin lesions showed hyperkeratosis, parakeratosis, and acanthotic epidermis with regular rete ridge elongation, consistent with psoriasis.
  • This case represents the first documented instance of Hyper IgE syndrome co-occurring with psoriasis disorder.

Implications:

  • The findings suggest a potential link between Hyper IgE syndrome and psoriasis.
  • Further research into the underlying mechanisms may elucidate novel therapeutic targets for both conditions.
