Physicians
The physician's primary responsibility is to diagnose illness and direct the medical or surgical treatment of the condition. The authority to admit patients to a healthcare agency or institution and practice care within that setting is granted to physicians by the healthcare agency or institution itself.

Planning Nursing Care I

The planning phase of the nursing process helps nurses set priorities, outline patient-centered goals and expected outcomes, and tailor nursing interventions to align with the aligned care plan. Through the planning phase, the nurse applies critical thinking skills to align and develop interventions according to the patient's needs. It provides continuity of care allowing patients to receive the maximum benefit from treatment. It serves as a pilot plan for allocating individual staff to a...
Nursing Implementation01:15

Nursing Implementation

Implementation is the execution of the nursing care plan developed during the planning phase.
The five steps to implementing effective nursing care include reassessing the patient, reviewing and revising the existing nursing care plan, organizing the resources and care delivery, anticipating and preventing complications, and implementing nursing interventions.
Discharge Summary Forms01:31

Discharge Summary Forms

The discharge summary is crucial as it enables a smooth transition from a healthcare facility to a patient's home or another care setting. This critical document facilitates seamless continuity of care, ensuring patients receive the necessary support and attention.
Here's a detailed look at the key components and guidelines for preparing a discharge summary:

Aneurysm III: Interprofessional Care

Aneurysm management involves either conservative medical therapy or surgical intervention, depending on the size and symptoms of the aneurysm. Conservative management is generally reserved for smaller, asymptomatic aneurysms, while larger or symptomatic aneurysms often necessitate surgical repair.Conservative Medical TherapyFor small, asymptomatic aneurysms, particularly abdominal aortic aneurysms (AAA) less than 5.5 centimeters in diameter, conservative medical therapy is recommended. Area of Science:

  • Health Professions Education
  • Interprofessional Collaboration
  • Patient Discharge Planning

Background:

  • Interprofessional education (IPE) is crucial for enhancing collaboration among healthcare professionals.
  • Effective discharge planning is essential for complex patient care, requiring coordinated efforts from diverse health professionals.

Purpose of the Study:

  • To introduce and evaluate an innovative interprofessional education (IPE) model utilizing a team-based discharge planning case scenario.
  • To assess the impact of a discharge planning simulation (DPS) on health professions students' perceptions of their roles and team collaboration.

Main Methods:

  • 173 occupational therapy, physician assistant, and physical therapy students participated in a discharge planning simulation (DPS).
  • The simulation focused on a complex patient case involving stroke and hip fracture.
  • Pre- and post-simulation surveys were administered to evaluate student perceptions.

Main Results:

  • High response rates for both pre- (89%) and post- (77%) surveys were achieved.
  • No significant differences in perceptions were found based on the students' program of study.
  • Significant changes in student perceptions were observed regarding their individual roles and the roles of their interprofessional team members.

Conclusions:

  • The discharge planning simulation (DPS) model led to significant positive changes in students' understanding of roles within an interprofessional team.
  • Preliminary findings suggest this IPE model is effective for teaching interprofessional teamwork in discharge planning.
  • The model holds promise for preparing future health professionals for collaborative patient care.
