Summary

Sumatriptan and venlafaxine can cause serotonin syndrome, a serious condition with symptoms like head twitching and muscle spasms. Promptly stopping these drugs and using benzodiazepines can resolve symptoms.

  • A 45-year-old male presented with head twitching and myoclonic movements.
  • The patient was on long-term venlafaxine for depression and self-medicating with sumatriptan for post-accident head pain.

Observation:

  • Cranial CT and cervical spine imaging ruled out trauma-related injuries.

Findings:

  • Symptoms resolved after discontinuing sumatriptan and venlafaxine, and administering lorazepam.
  • This case highlights the potential for drug interactions leading to serotonin syndrome.

Implications:

  • Serotonin syndrome requires high diagnostic suspicion, especially in patients on multiple serotonergic agents.
