Background:

  • Disability assessment in the Netherlands occurs after two years of sick leave, a point where functioning may still evolve.
  • Accurate prognosis of claimant functioning is challenging for professionals.
  • Self-predictions by claimants may offer greater accuracy than professional assessments.

Purpose of the Study:

  • To evaluate and compare the predictive accuracy of functioning improvement between disability claimants and insurance physicians (IPs).
  • To investigate if prediction accuracy differs between claimants with mental versus somatic health conditions.

Main Methods:

  • A prospective cohort study involving 375 disability claimants assessed at baseline and 276 after one year.
  • Functioning data collected via self-report (WHO Disability Schedule 2.0).
  • Predictions of improvement by claimants and IPs assessed using sensitivity, specificity, and AUC; mixed logistic regression used for subgroup analysis.

Main Results:

  • One-third (32%) of claimants showed functional improvement beyond measurement error.
  • Both claimants and IPs demonstrated limited ability to predict this improvement, with AUCs of 0.62 and 0.61, respectively.
  • No significant differences in prediction accuracy were observed between claimants with mental or somatic health conditions.

Conclusions:

  • Functional improvement is a common outcome for disability claimants.
  • Neither insurance physicians nor disability claimants could accurately predict functional improvement in either mental or somatic health conditions.
