Background:

Leiomyomas are common benign uterine smooth muscle tumors.
Intravenous leiomyomatosis (IVL) is a rare variant characterized by intraluminal vascular invasion.
IVL can extend into pelvic veins, potentially reaching the inferior vena cava and right atrium.

Observation:

This report details a case of uterine intravenous leiomyomatosis managed at our institution.
The condition presented as worm-like tumor protrusions within veins at the parametrial margins.
Vascular involvement outside the primary myoma defines this entity.

Findings:

Diagnosis of intravenous leiomyomatosis requires careful examination of surgical specimens.
Histopathological analysis confirms the benign smooth muscle origin and vascular invasion.
