The uterine wall consists of three histological layers: the perimetrium, myometrium, and endometrium. Background:

  • Histological chorioamnionitis (HCA) is an intra-amniotic infection.
  • The relationship between HCA grades and gestational age requires further elucidation.

Purpose of the Study:

  • To investigate the association between different grades of HCA and gestational age at delivery.
  • To analyze HCA distribution in term and preterm deliveries.

Main Methods:

  • Enrolled 392 women with spontaneous labor and HCA diagnosis.
  • Classified HCA into three grades (HCA1, HCA2, HCA3) based on inflammation location.
  • Performed microbiological cultures on placental and fetal membrane samples.

Main Results:

  • HCA1 was more frequent in term deliveries (p < 0.001).
  • HCA2 was more common in deliveries between 32-36 weeks (p < 0.001).
  • HCA3 occurred more frequently in deliveries within 32 weeks (p < 0.001).
  • Positive bacterial cultures were higher in HCA3 compared to HCA1 and HCA2 (p = 0.008).

Conclusions:

  • HCA grade distribution significantly differs based on gestational age at delivery.
  • HCA may reflect varying etiologies and gestational tissue immune competence.
  • Findings suggest HCA grading is a relevant indicator in relation to pregnancy duration.
