Summary

Generalized gingival enlargement can be a rare initial sign of multiple myeloma (a plasma cell cancer). This case highlights the importance of investigating systemic diseases like multiple myeloma in patients with unexplained gingival hyperplasia.

Background:

  • Multiple myeloma is a plasma cell malignancy.
  • Oral manifestations are uncommon initial symptoms.
  • Gingival enlargement is a rare presentation.

Observation:

  • A 58-year-old male presented with 6 months of generalized gingival enlargement.
  • Initial clinical diagnosis was gingival hyperplasia.
  • Histopathology revealed plasma cell infiltration.

Findings:

  • Biopsy showed stratified squamous epithelium with ulceration.
  • Subepithelial infiltration by binucleate and multinucleate plasma cells.
  • Cytoplasmic inclusion bodies were noted in plasma cells, leading to a multiple myeloma diagnosis.

Implications:

  • Gingival hyperplasia can be an early indicator of multiple myeloma.
  • Systemic workup is crucial for unexplained gingival enlargement.
  • Early diagnosis of multiple myeloma improves patient outcomes.
