Summary

Microscopic malaria diagnosis using Giemsa-stained slides showed excellent agreement for negative or high parasite counts. However, low parasite counts (1-100 parasites/μl) require careful re-examination for accurate Plasmodium falciparum diagnosis.

Background:

Purpose of the Study:

  • To evaluate the consistency of malaria parasite detection by different observers using Giemsa-stained slides.
  • To determine the reliability of microscopic malaria diagnosis in a field setting.

Main Methods:

  • Children (0-10 years) in Bancoumana, Mali, were enrolled during transmission seasons.
  • Blood smears from persistently negative children were re-examined.
  • A stratified random sample of slides based on parasite density was analyzed.
  • Kappa statistics and intra-class correlation coefficients measured inter-observer agreement.

Main Results:

  • Excellent inter-observer agreement was observed overall (weighted kappa statistic κ(w)=0.7926).
  • High agreement was found for negative slides (97%) and slides with >100 parasites/μl (92%).
