Late-onset encapsulating peritoneal sclerosis (EPS) can occur years after kidney transplantation, even without inflammation or CT scan abnormalities. Surgical exploration is crucial for diagnosis when clinical suspicion is high.

Background:

  • Encapsulating peritoneal sclerosis (EPS) is a serious complication post-kidney transplantation, particularly in patients with prior peritoneal dialysis.
  • Systemic inflammation and CT scan findings are typical indicators of EPS.

Observation:

  • A 59-year-old female presented with bowel obstruction 33 months post-kidney transplant.
  • She had a history of extensive peritoneal dialysis.
  • Physical exam and labs showed no systemic inflammation; CT scan was inconclusive for EPS.

Findings:

  • Despite negative inflammatory markers and normal CT scan, laparotomy revealed classical EPS.
  • The patient underwent peritonectomy and enterolysis, resulting in restored peristalsis.
