    Background:

    • Celiac disease (CD) is an autoimmune disorder affecting children.
    • Accurate assessment of CD activity is crucial for effective management.
    • Current diagnostic methods can be invasive.

    Purpose of the Study:

    • To investigate beta-2 microglobulin (beta2M) as a potential noninvasive biomarker for celiac disease activity in children.
    • To compare beta2M levels in children with celiac disease to those with other causes of chronic diarrhea and healthy controls.

    Main Methods:

    • Sera from 36 children with celiac disease and 19 children with other chronic diarrhea causes were analyzed over 4 years.
    • Beta-2 microglobulin (beta2M), orosomucoid, alpha1-antitrypsin, and C-reactive protein (CRP) levels were measured.

  • Intestinal biopsies were performed to classify celiac disease status (inactive vs. active).
  • Main Results:

    • Beta2M levels were significantly higher in children with active celiac disease (2.58 +/- 0.76 mg/L) compared to inactive CD (1.94 +/- 0.49 mg/L), controls (1.87 +/- 0.49 mg/L), and other chronic diarrhea (1.72 +/- 0.63 mg/L).
    • Elevated beta2M was found in 87.5% of newly diagnosed CD children versus 15.7% of non-celiac patients.
    • Serial studies showed the highest beta2M levels correlated with flat mucosa (intestinal villous atrophy) in 10 out of 12 CD children.

    Conclusions:

    • Elevated beta2M levels are associated with active celiac disease in children.
    • Beta2M may serve as a useful noninvasive marker for assessing celiac disease activity and monitoring treatment response.
    • Further research could explore the link between beta2M, intestinal inflammation, and lymphocyte activation in celiac disease.
