Economy in transurethral prostatectomy.

H H Meyhoff, J Nordling, T Hald

Scandinavian Journal of Urology and Nephrology
    |January 1, 1985
    PubMed
    Summary

Transurethral (TURP) prostatectomy is more economical than transvesical (TVP) prostatectomy for benign prostatic hyperplasia, reducing recovery time and healthcare costs. TURP offers significant advantages in patient recovery and resource utilization.

    Area of Science:

    • Urology
    • Health Economics

    Background:

    • Benign prostatic hyperplasia (BPH) is a common condition in aging men.
    • Surgical interventions like transurethral (TURP) and transvesical (TVP) prostatectomy are standard treatments.
    • Evaluating the economic impact of different surgical approaches is crucial for healthcare resource allocation.

    Purpose of the Study:

    • To compare the economic outcomes of transurethral prostatectomy (TURP) versus transvesical prostatectomy (TVP).
    • To assess differences in resource utilization and patient recovery between TURP and TVP for medium-sized prostates.

    Main Methods:

    • A prospective randomized study comparing TURP and TVP.
    • Inclusion of 43 patients for TURP and 32 for TVP.
    • Evaluation of postoperative ward time, anesthesia duration, antibiotic use, IV infusions, outpatient visits, readmissions, and sick leave.

    Main Results:

    • TURP demonstrated significantly shorter median postoperative ward time (7 days vs. 15.5 days for TVP).
    • TURP required less anesthesia, antibiotics, and IV fluids, with fewer outpatient visits.
    • No significant difference in hospital readmission rates within 6 months was observed between the groups.
    • Sick leave was shorter for TURP patients (2 weeks vs. 4 weeks for TVP).

    Conclusions:

    • TURP is a more cost-effective procedure for medium-sized prostatic hyperplasia compared to TVP.
    • Increased adoption of TURP could reduce waiting lists and yield substantial cost savings for the healthcare system.
    • The findings suggest a significant potential for economic retrenchment through wider implementation of TURP.
