Background:

Purpose of the Study:

  • To evaluate an image analysis method for differentiating allergic and irritant patch test reactions.
  • To quantify erythema at patch test sites using digital imaging.

Main Methods:

  • 172 Japanese patients underwent patch testing with sodium lauryl sulfate (SLS) and nickel sulfate.
  • Digital photography and visual evaluation were performed at 48 and 72 hours.
  • Erythema Index (EI) images were generated, and changes in ΔEI were analyzed.

Main Results:

  • A significant increase in ΔEI from 48 to 72 hours was observed for nickel sulfate (allergic) reactions (P < 0.0001).
  • No significant difference in ΔEI was found for SLS (irritant) reactions between 48 and 72 hours.

Conclusions:

  • Image analysis of erythema changes over time can distinguish allergic from irritant patch test reactions.
  • The method provides an objective approach to patch test interpretation.
