Peripheral atherosclerosis in patients with arterial erectile dysfunction.

Peripheral atherosclerosis is linked to arterial erectile dysfunction (ED). Men with ED should be screened for conditions like diabetes and hypertension to detect silent vascular disease and prevent complications.

Purpose of the Study:
- To investigate the association between penile cavernosal arterial blood flow and peripheral atherosclerosis in men diagnosed with ED.
- To determine if markers of systemic atherosclerosis correlate with arterial ED.
Main Methods:
- 102 patients with suspected ED underwent penile color Doppler ultrasound (CDUS) to assess cavernosal artery flow.
- Carotid and femoral artery intima-media thickness and atherosclerotic plaques were evaluated using B-mode ultrasonography.
- Fasting blood glucose, total cholesterol, and HDL levels were analyzed; history of diabetes, hypertension (HT), and smoking were recorded.
Main Results:
- 42.2% of patients had arterial ED based on penile CDUS findings.
- Significant differences were observed between arterial ED and non-arterial ED groups regarding diabetes mellitus (DM), HT, atherosclerotic cardiovascular diseases (ACVD), and total cholesterol levels (P<0.05).
- The presence of atherosclerotic plaque in the carotid and femoral arteries was significantly higher in the arterial ED group (P<0.05). Smoking showed no statistically significant relationship with arterial ED (P>0.05).
Conclusions:
- Undiagnosed or subclinical DM, HT, and ACVD may contribute to the etiology of arterial ED.
- Patients diagnosed with arterial ED via Doppler Ultrasound warrant further evaluation for peripheral vascular atherosclerosis using B-mode ultrasonography.
- Early detection of subclinical atherosclerosis in ED patients can help prevent future cardiovascular complications.
