Background:

  • Nephron sparing surgery (NSS) is the recommended treatment for T1 renal cell carcinoma (RCC) when feasible.
  • Current practice regarding NSS in older RCC patients requires broader evaluation.

Purpose of the Study:

  • To investigate if current surgical practices for RCC in Lower Saxony, Germany, align with the need for NSS in elderly patients.
  • To analyze the rates of partial nephrectomy (PN) versus radical nephrectomy (RN) in different age groups.

Main Methods:

  • Data from over 100 urological clinics in Lower Saxony, Germany (2005-2010) were analyzed.
  • Individual rates of PN and RN were assessed based on 5-year age intervals for RCC patients.
  • 3,332 patient records with available age data were included in the analysis.

Main Results:

  • Overall PN rates were 19.9% for all RCC patients and 29.5% for T1 RCC patients.
  • PN rates were significantly lower in patients above the median age (17.8% for all RCC, 26.4% for T1 RCC) compared to those below the median age (21.9% for all RCC, 32.6% for T1 RCC).
  • The highest rates for RN in T1 RCC patients were observed in the 70-75 age group.

Conclusions:

  • The utilization of PN for RCC, particularly T1 RCC, is significantly reduced in older patient populations.
  • This suggests a potential gap in the understanding and application of NSS for elderly RCC patients.
  • Enhanced education and training in NSS are recommended to improve treatment outcomes for older adults with RCC.
