Background:

  • Endobronchial involvement by extrapulmonary malignancies is rare.
  • Commonly associated cancers include breast, kidney, colon, and rectum carcinomas.

Purpose of the Study:

  • To report a rare case of endobronchial recurrence of sigmoid colon non-Hodgkin lymphoma (NHL).
  • To describe the diagnostic and therapeutic approach for this unusual presentation.

Main Methods:

  • A 68-year-old male with a history of colon NHL presented with cough, sputum, and dyspnea.
  • Diagnostic workup included chest radiography, CT scan, and fiberoptic bronchoscopy with biopsy.
  • Treatment involved R-ICE chemotherapy (rituximab, ifosfamide, carboplatin, etoposide).

Main Results:

  • Imaging revealed right hilar enlargement, a right middle zone opacity, and a mass invading the thoracic wall with rib destruction.
  • Bronchoscopy showed bronchial stenosis with tumor infiltration in the right upper lobe.
  • Pathology confirmed diffuse large B-cell lymphoma, consistent with endobronchial recurrence.
  • Partial regression of pulmonary lesions was observed after three cycles of R-ICE chemotherapy.

Conclusions:

  • Endobronchial recurrence of colon NHL is an uncommon but possible manifestation.
  • Multidisciplinary diagnostic evaluation and chemotherapy are crucial for managing such cases.
  • R-ICE chemotherapy showed partial efficacy in this patient.
