A rare giant left ventricular aneurysm, despite its size and proximity to the mitral valve, did not impair valve function. Surgical ventricular restoration was successfully performed in this unique case.

- This case highlights the possibility of intact mitral valve function even with massive LVAs in close proximity.
- Successful surgical repair of giant LVAs is feasible, offering a potential treatment option for selected patients.
