Background:

• Extramammary Paget's Disease (EMPD) is a rare intraepithelial adenocarcinoma.

Observation:

• An 83-year-old male presented with an 8-year history of a pruritic, erythematous plaque on his left axilla.
• Previous treatments with topical antifungals and steroids provided only temporary relief, with progressive disease.

Findings:

• Clinical and histological examination confirmed Extramammary Paget's Disease.
• The patient underwent a total excision with wide margins.

Implications:

• This case highlights a rare presentation of EMPD in a male axilla.
• Complete surgical resection resulted in a disease-free status at 12 months post-operation, suggesting curative potential for localized EMPD.
