Diffusion tensor imaging (DTI) reveals white matter changes after stroke. A novel voxel-wise connectivity metric effectively predicts upper extremity motor function in stroke survivors.

Purpose of the Study:

  • To objectively assess stroke lesion effects on white matter structure using DTI.
  • To evaluate DTI metrics for predicting sensorimotor function in stroke survivors.
  • To introduce a voxel-based approach for assessing stroke's global impact on motor function.

Main Methods:

  • Acquired T1-weighted and DTI brain images from 10 stroke survivors and 9 controls.
  • Employed a voxel-wise indirect structural connectivity (VISC) index derived from DTI tractography.
  • Performed voxel-based analyses comparing VISC, fractional anisotropy (FA), and mean diffusivity (MD) with Fugl-Meyer (FM) scores.

Main Results:

  • VISC (R2=0.796) and FA (R2=0.674) showed stronger correlations with total FM scores than MD (R2=0.512).
  • Correlations were primarily driven by upper extremity motor function (VISC: R2=0.879).
  • FA showed less influence from lower extremity function (R2=0.177).

Conclusions:

  • Voxel-wise DTI metrics, particularly VISC, can predict upper extremity sensorimotor function post-stroke.
  • Supraspinal intraconnectivity appears less critical for lower extremity function recovery.
  • This DTI-based approach offers a promising tool for assessing stroke recovery.
