    Summary

    This 3-year study shows that immediately loaded zygomatic implants provide successful rehabilitation for patients with severe maxillary atrophy. Dental prostheses supported by these implants demonstrated high success rates and improved quality of life.

    Background:

    • Severe maxillary atrophy presents significant challenges for dental rehabilitation.
    • Traditional implant-supported prostheses may not be suitable for all cases of severe atrophy.
    • Zygomatic implants offer a potential solution for complex maxillary reconstructions.

    Purpose of the Study:

    • To evaluate the 3-year clinical outcomes of dental prostheses supported by four immediately loaded zygomatic implants.
    • To assess the success rates of both the prostheses and the zygomatic implants.
    • To document complications and changes in oral-health related quality of life (OHRQoL).

    Main Methods:

    • A prospective study involving 17 patients with severe maxillary atrophy.
    • Immediate rehabilitation with prostheses supported by four immediately loaded zygomatic implants.
    • Follow-up for 3 years, assessing prosthesis and implant success, complications, and OHRQoL using the OHIP-14 questionnaire.

    Main Results:

    • No dropouts; 100% survival rate for prostheses and zygomatic implants (one implant not used due to unfavorable emergence).
    • Seven complications reported: orbital penetration (no consequences), infection/fistula (resolved), sinusitis (resolved), abutment screw fracture (one patient), prosthesis fracture (two patients, repaired).
    • Average OHIP-14 score was 2.7, comparable to the general population, indicating good OHRQoL.

    Conclusions:

    • Four immediately loaded zygomatic implants can successfully rehabilitate patients with severe maxillary atrophy.
    • This treatment modality demonstrates high implant and prosthesis survival rates over 3 years.
    • Patients experience favorable oral-health related quality of life following rehabilitation with zygomatic implants.
