Summary

Laparoscopic partial nephrectomy (LPN) offers comparable oncologic and functional outcomes to open partial nephrectomy (OPN) for renal masses. While LPN showed shorter surgical times and blood loss, long-term data is still needed.

Background:

Purpose of the Study:

  • To compare surgical, oncologic, and functional outcomes of LPN versus OPN for renal masses.
  • To evaluate intraoperative, postoperative, and long-term results of both partial nephrectomy techniques.

Main Methods:

  • Retrospective comparison of 115 LPNs and 97 OPNs performed between 2006 and 2011.
  • Matched patient demographics and analyzed surgical time, blood loss, complications, and functional outcomes (GFR).
  • Assessed oncologic outcomes using local recurrence-free survival via Kaplan-Meier estimates over a mean follow-up of approximately 30 months.

Main Results:

  • LPN group demonstrated significantly shorter surgical time, occlusion time, blood loss, and hospitalization (p < 0.01).
  • Total complications were similar, though LPN had more intraoperative complications (subcutaneous emphysema).
  • Oncologic outcomes were comparable (60-month local recurrence-free survival: 92.4% LPN vs. 93.8% OPN, p=0.57); GFR reduction was transiently greater after LPN but similar long-term.

Conclusions:

  • Laparoscopic partial nephrectomy (LPN) is a viable alternative to open partial nephrectomy (OPN) for renal masses, yielding similar oncologic and functional results.
  • LPN offers advantages in reduced surgical morbidity.
  • Further long-term studies are necessary to definitively establish oncologic and functional equivalence.
