Daptomycin and vancomycin show similar effectiveness and safety for treating methicillin-resistant Staphylococcus aureus osteoarticular infections (MRSA OAIs). Daptomycin offers a viable alternative, especially when vancomycin is not well-tolerated.

- Vancomycin is the standard treatment for methicillin-resistant Staphylococcus aureus (MRSA) infections.
- Daptomycin is approved for MRSA bacteremia, but its efficacy in osteoarticular infections (OAIs) remains unestablished.
Purpose of the Study:
- To compare the clinical outcomes and drug toxicity of daptomycin versus vancomycin in adult patients with MRSA OAIs.
- To evaluate daptomycin as an alternative treatment for MRSA OAIs.
Main Methods:
- A 1:2 nested case-control study was conducted from 2005 to 2010.
- Twenty patients treated with daptomycin were matched to 40 patients treated with vancomycin for MRSA OAIs.
- Clinical success rates and adverse events were compared between the two groups.

- Clinical success rates at 3 and 6 months were similar between daptomycin (75% and 70%) and vancomycin (68% and 58%) groups.
- The frequency of adverse events was also comparable between the daptomycin (5%) and vancomycin (18%) groups.
- Diabetic patients were more likely to receive vancomycin, and vancomycin was more often combined with other antibiotics.
Conclusions:
- Daptomycin demonstrates similar clinical success and tolerability compared to vancomycin for MRSA OAIs.
- Daptomycin represents a reasonable alternative for treating MRSA OAIs, particularly in patients intolerant to vancomycin.
