Background:

  • Sarcoidosis is a multisystem inflammatory disease.
  • Parotid gland involvement is a rare manifestation of sarcoidosis.

Observation:

  • A 32-year-old woman presented with bilateral parotid gland enlargement and long-standing bilateral blindness.
  • Physical examination revealed bilateral firm, painless parotid masses.
  • Imaging showed pulmonary nodules and enlarged mediastinal/axillary lymph nodes.

Findings:

  • Histopathology of the parotid gland revealed epithelioid noncaseating granulomas, consistent with sarcoidosis.
  • The patient had no systemic symptoms like fever, cough, or weight loss.

Implications:

  • Painless parotid gland swelling, especially in women, warrants consideration of sarcoidosis.
  • This presentation may represent an early sign of sarcoidosis.
  • Ophthalmologic and pulmonary evaluations are crucial in cases of unexplained parotid enlargement.
