Urologic Endoscopic Procedure: Cystoscopic Examination

6.6K
Meaning of Cystoscopic Examination:Cystoscopy is an essential diagnostic tool in urology that is used to assess the structure and function of the genitourinary system. It provides a direct view of the urethra, bladder, and, in some cases, the ureteral openings. This procedure helps detect structural abnormalities, infections, cancers, and blockages in the urinary tract. There are two types of cystoscopy:Flexible cystoscopy is commonly performed in outpatient settings due to its less invasive...

Summary

Cloacal exstrophy management in a resource-limited setting showed high mortality due to prematurity and late presentation. Single-stage reconstruction without osteotomy yielded satisfactory outcomes for this complex anomaly.

Area of Science:

  • Pediatric Surgery
  • Congenital Anomalies
  • Neonatal Care

Background:

  • Cloacal exstrophy is a rare and complex congenital anomaly requiring specialized management.
  • Tertiary care centers face challenges in managing such cases, especially with limited infrastructure.

Purpose of the Study:

  • To analyze the anatomical types, associated anomalies, surgical procedures, and outcomes of cloacal exstrophy patients.
  • To identify factors contributing to mortality and evaluate the effectiveness of surgical approaches.

Main Methods:

  • Retrospective analysis of 23 patients with cloacal exstrophy treated at a tertiary care center.
  • Detailed recording of patient demographics, anomaly presentation, and surgical interventions.
  • Evaluation of primary and secondary surgical repair outcomes, including bowel function.

Main Results:

  • 19 out of 23 patients had associated anomalies; 17 were preterm with low birth weight.
  • High mortality (5 preoperatively, 5 postoperatively) was linked to major anomalies, prematurity, and sepsis.
  • Single-stage surgical reconstruction was performed in 15 patients, with some awaiting further procedures.

Conclusions:

  • Prematurity, late presentation, and sepsis are critical factors influencing high mortality rates in cloacal exstrophy.
  • Single-stage reconstruction without osteotomy demonstrated satisfactory results in this patient cohort.
