    Background:

    • The diagnosis of word deafness has evolved, leading to confusion with related terms like pure word deafness, auditory agnosia, and cortical deafness.
    • This study aims to clarify the diagnostic landscape of word deafness and related cortical auditory disorders.

    Observation:

    • Three new cases of word deafness, including one with CT and necropsy correlation, were analyzed.
    • These cases were compared with 34 previously reported cases of cortical auditory disorders.
    • Patients with word deafness consistently exhibited pervasive auditory agnosia, affecting both linguistic and non-linguistic sound comprehension, as well as musical abilities.

    Findings:

    • Word deafness encompasses a spectrum of auditory deficits and language abnormalities, but shares common features in etiology, pathology, clinical presentation, and course.
    • The term "word deafness" is retained due to the distinctive deficit in comprehending spoken words.
    • The most frequent cause is cerebrovascular accidents, often from cardiac emboli, leading to bitemporal cortico-subcortical lesions.

    Implications:

    • The findings support unifying heterogeneous cortical auditory disorders under the umbrella term "word deafness".
    • Cerebrovascular accidents are the primary etiology, with bitemporal lesions being common.
    • The severity of auditory impairment, including musical deficits, correlates with the extent of the disorder.
