    Summary

    In ulcerative colitis (UC) patients, rectal epithelial cells were HLA-DR-positive initially. After 3 months, some patients became HLA-DR-negative, but this change wasn't linked to disease severity.

    Background:

    • Ulcerative colitis (UC) is a chronic inflammatory bowel disease.
    • Human Leukocyte Antigen - antigen D Related (HLA-DR) is typically expressed on immunocompetent cells.
    • The presence of HLA-DR on non-immune cells in UC is not fully understood.

    Purpose of the Study:

    • To investigate the expression of HLA-DR antigens on rectal epithelial cells in patients with active ulcerative colitis.
    • To determine if HLA-DR expression correlates with clinical, rectoscopic, or histologic disease activity.
    • To observe changes in HLA-DR expression over a 3-month period.

    Main Methods:

    • Immunohistochemical analysis of rectal epithelial cells for HLA-DR antigens.
    • Assessment of clinical, rectoscopic, and histologic disease stages.
    • Serial examinations at baseline, 2 weeks, and 3 months.

    Main Results:

    • All 12 UC patients exhibited HLA-DR-positive rectal epithelial cells at baseline and 2 weeks.
    • After 3 months, 5 patients became HLA-DR-negative, while 7 remained HLA-DR-positive.
    • No correlation was found between HLA-DR expression/nonexpression and clinical, rectoscopic, or histologic findings.

    Conclusions:

    • Rectal epithelial cells in active UC patients can express HLA-DR antigens.
    • The dynamic expression of HLA-DR on these cells may reflect underlying immunological activity.
    • The functional significance of HLA-DR-positive epithelial cells in UC remains to be elucidated.
