    Summary

Surgery offers the only cure for pancreatic islet cell tumors. Surgical outcomes vary by tumor type, with insulinomas showing the highest 10-year survival rates, emphasizing the importance of surgical intervention.

    Area of Science:

    • Endocrinology
    • Surgical Oncology
    • Gastroenterology

    Background:

    • Pancreatic islet cell tumors are diverse, including functioning and nonfunctioning types.
    • These tumors can cause specific syndromes or be asymptomatic.

    Purpose of the Study:

    • To review surgical outcomes for pancreatic islet cell tumors.
    • To analyze tumor types, localization, surgical techniques, and survival rates.

    Main Methods:

    • Retrospective review of 84 patients with pancreatic islet cell tumors operated on over 35 years.
    • Analysis of tumor types (nonfunctioning, insulinomas, gastrinomas, VIP-omas, carcinoids, parathyrinomas), patient demographics, and surgical procedures.
    • Evaluation of preoperative localization success rates, intraoperative localization techniques, and surgical approaches (distal pancreatectomy, enucleation, Whipple procedure).

    Main Results:

    • Tumor types included nonfunctioning (21), insulinomas (41), gastrinomas (16), VIP-omas (2), carcinoids (2), and parathyrinomas (2).
    • Eleven patients had Multiple Endocrine Neoplasia type I syndrome.
    • Preoperative localization was successful in 63% of attempted cases.
    • Operative morbidity was 24%, and mortality was 3.6%.
    • 10-year survival rates: nonfunctioning (54.7%), gastrinomas (68.4%), insulinomas (92.4%).

    Conclusions:

    • Surgery is the only curative treatment for pancreatic islet cell tumors.
    • Surgical approach depends on tumor location, size, and malignancy.
    • Tumor type significantly impacts long-term survival, with insulinomas demonstrating excellent prognosis post-surgery.
