    Summary

    Primary pulmonary artery leiomyosarcoma is rare, with few surgical cases reported. Surgical intervention in obstructive shock offered only temporary relief, followed by fatal septic shock.

    Background:

    • Primary leiomyosarcoma of the pulmonary artery is an exceptionally rare tumor.
    • Literature review reveals only 12 previously reported cases.
    • Surgical treatment has been attempted in a minority of these rare instances.

    Observation:

    • A patient presented with obstructive shock due to pulmonary artery leiomyosarcoma.
    • Surgical excision was performed using extracorporeal circulation.
    • The patient experienced temporary hemodynamic improvement post-surgery.

    Findings:

    • Despite surgical intervention, the patient succumbed to septic shock on the 10th postoperative day.
    • This case highlights the challenges in managing advanced pulmonary artery leiomyosarcoma.
