Summary

Ciprofloxacin, an antibiotic, may cause syndrome of inappropriate antidiuretic hormone (SIADH), leading to dangerously low sodium levels. This case highlights the potential fluoroquinolone-induced SIADH risk.

Background:

  • Syndrome of inappropriate antidiuretic hormone (SIADH) is a condition characterized by excessive antidiuretic hormone release, leading to water retention and hyponatremia.
  • Fluoroquinolones are broad-spectrum antibiotics commonly used for urinary tract infections.

Observation:

  • A 68-year-old woman experienced recurrent generalized weakness on two separate occasions, both times after initiating ciprofloxacin for a urinary tract infection.
  • Upon ciprofloxacin discontinuation during hospitalization, her serum sodium levels rapidly increased, indicating a potential drug-induced effect.

Findings:

  • A causality assessment using the Naranjo scale indicated a probable link between ciprofloxacin use and the development of SIADH (score of 8).
  • The proposed mechanism involves ciprofloxacin crossing the blood-brain barrier, stimulating neurotransmitter receptors (GABA and NMDA), and subsequently increasing antidiuretic hormone synthesis and release.
